v2.9.1 — Four small debts, one sweep

Patch release. No migrations.

• The mapping checkbox that read “On” now reads Assemble fallback with a tooltip — it was the assemble flag, not a mapping switch, and the label misled the operator.
• B2B scan matchers are exact (pick location, pack package, ship reference): A-1 no longer verifies A-1-1-1 anywhere. Specs assert the refusals.
• Station console workbench stays on screen: the line list scrolls; scan field and named errors are pinned inside the viewport, asserted by geometry at 1440×900.
• Channel hygiene stops calling dead integrations: AUTH_DEAD short-circuits before any remote call (alert stays open, log at debug); saving credentials re-arms verification.
